Diabetes Programmes
Diabetes is a signi昀椀cant cause
of mortality and morbidity in
Te Tai Tokerau. Early detection
and reversal of pre-diabetes
can reduce the harm caused by
the disease and prevent other
associated chronic conditions
and co-morbidities, such as
gout and heart disease, from
developing.
Type 2 Diabetes Libre
Sensor Trial
This joint project between
Mahitahi Hauora, general practice
Te Ara Tu o Ngāti Hine, Ngāti Hine
Health Trust and the diabetic
team at Health New Zealand | Te
Whatu Ora – Te Tai Tokerau began
in August 2024.
Patients of Te Ara Tu o Ngāti
Hine newly diagnosed with
Type 2 Diabetes were offered a
Mahitahi Hauora-funded Libre
sensors to monitor their blood
sugar levels and enrolled in a
10-week Te Ao Māori support
programme, provided by Ngāti
Hine Health Trust, and focussing
on lifestyle improvements.
Comparing the blood sugar levels
(HbA1c) of patients before and
after they completed the support
programme, 70% of patients saw
an increase in their ‘time in target’
range (between 3.9-10 mmol).
Tauparetia Te Mate
Hukahuka Programme
Following on from the
pre-diabetes pilot that began
in July 2024, the team at
Whakapiri Ora developed a new
Kaupapa Māori health coaching
programme. Concluding in
December 2024, the programme
produced positive results,
65% of those who engaged
reduced their HbA1c levels,
and 70% reduced their waist
circumference.
