Long Term Conditions
(LTCs) significantly
contribute towards
poor health and
early death and are
especially prevalent
among Māori.
Mahitahi Hauora leads service
delivery projects targeting
chronic conditions where
we see inequitable health
outcomes. Our LTC programme
supports prevention, early
detection, early intervention,
and the overall wellbeing of
patients living with chronic
conditions in Te Tai Tokerau.

Asthma Whānau
Wellness Checks
Following the success of
Nurse-led COPD Whānau
wellness checks launched in
2024, in February 2025 we
launched Nurse-led Asthma
Whānau Wellness checks,
providing additional funding and
resources for general practices.
This expands on our longterm conditions programme’s
respiratory services which also
include spirometry coordination
services.
Gout Awareness
From September 2024, Mahitahi
Hauora and Arthritis New Zealand
co-funded a series of radio ads
to play across radio stations
reaching Māori listeners in
Whangārei, Okaihau and Kaitaia.
The campaign also included
boosted posts on Ngāti Hine FM,
Tautoko FM and Te Hiku o Te Ika’s
Facebook pages, which reached
almost 4,000 people resulting in
over 1,000 clicks.
Mahitahi Hauora shared our Gout
Stop Programme data and data
on the rates of chronic conditions
amongst our enrolled Paci昀椀c
population with The Northern
Region Paci昀椀c Gout Network
to help illustrate how gout
disproportionately affects Paci昀椀c
people in Northland. We also
engaged with a research team at
University of Auckland, who were
seeking to understand how gout
programmes in Northland help
Paci昀椀c people.
