Living Well
We are focussing on
work programmes that
support access to care
in the community with
the aim of enabling
whānau to live well.
This includes supporting
our partner practices to
develop and sustain the
delivery of services that meet
the needs of their enrolled
patients, at a time when the
health system is grappling with
increased community demand,
alongside workforce and
funding issues.
The Partnership Services
Team work with general
practice offering support
around compliance with
Foundation Standards, PMS
support, contract reporting,
providing clinical, operational
and sector updates, supporting
the implementation and
enablement of new workforce
via the Comprehensive Primary
and Community Teams (CPCT),
and with funding for priority
groups such as Long-Acting
Reversible Contraceptives
(LARC), cervical screening, and
through the Primary Options
Acute Demand Management
Service (POADMS).

Comprehensive
Primary and
Community Teams
The Comprehensive Primary
and Community Teams (CPCT)
initiative received funding for
frontline clinical roles that sit
within general practice, increasing
capacity and improving access
to care for people with complex
conditions and high needs.
These roles, which include Care
Coordinators, Pharmacists,
Physiotherapists, Emergency Care
Paramedics, Physician Associates
and Kaiāwhina, help whānau to
receive early interventions and
faster treatment and provides
them with better support for social
and lifestyle factors that impact
their health.
This year, we have continued
with the recruitment work that
began last 昀椀nancial year and
have rolled out orientation and
interdisciplinary team training
programmes to support change
and integration of these roles into
the practice model of care.
Of the 26 eligible member general
practices, originally 19 chose to
participate in the initiative. There
are currently 32 CPCT roles (14.52
FTE) across 18 Mahitahi Hauora
member general practices.
