Discharges
This year we continued to work
alongside the Northland hospitals
Paediatrics teams, receiving
noti昀椀cations when a child who
was admitted with a respiratory
disorder is discharged from
hospital and has not met the
criteria for Paediatric Outreach
Nurse support.
Mahitahi Hauora’s Clinical
Hub nurses contact whānau to
discuss the hospital discharge
plan, medications and relevant
assessments, answering questions
and referring to other relevant
services if appropriate.
Pre-calling Hapū

Nurses pre-call women in Te
Tai Tokerau from 26 weeks into
their pregnancies to encourage
uptake of Pertussis and Influenza
immunisations.
Best Start Kōwae
This year, we continued our
partnership with The National
Hauora Coalition to offer Best
Start Kōwae pregnancy and postnatal assessment tools in most
of our member general practices.
The aim of the Best Start Kōwae
tools is to support consistent,
comprehensive, and equitable
care of pregnant people and
babies.
